massive patchqueue reorg

patches can now be anywhere and tagged patch to show up on the patch list.
Moved all the patchqueue stuff to todo items; some of it was merged into
existing todo items.

Close this one, I think I've explained fairly convincingly why the old
behavior was a bug, and the current behavior good sense.

move some bugs to todo items

add a wishlist tag, that can be used to separate some todos off onto a
wishlist page

add warnings about using map, inline, etc with the sidebar causing
performance nightmares

* Add a htmlext config option, this can be used to make ikiwiki generate
  html files with a different extension.

* Make the map plugin notice when pages in the map are deleted and update
  it.
